What Is NDIS Support Coordination?
Support Coordination is an NDIS-funded service that helps you make the most of your NDIS plan. At My PlanWell, we act as your guide through the NDIS system. Our team helps you understand your plan, connect with the right providers, and access community, mainstream, and government services. We also build your confidence and skills, so you can coordinate your own supports more effectively.
Through ongoing collaboration, we create a support network that aligns with your goals and needs, helping you take control of your NDIS journey.

Levels of Support Coordination at My PlanWell
There are three recognised levels of support coordination under the NDIS. At My PlanWell, we offer Level 1: Support Connection and Level 2: Coordination of Supports.
Level 1 – Support Connection
We help you understand and navigate your NDIS plan, connect with providers, and use community and mainstream services effectively. Our aim is to make sure your supports are organised and aligned with your needs.
Level 2 – Coordination of Supports
At this level, we help you build confidence in managing your plan. We connect you with services, negotiate with providers, and assist in gathering information when requesting plan reviews. Our coordinators also help you monitor your plan spending and explore different plan management options.
